"Please see the attached consent form for the Pfizer vaccine. If a parent or guardian is not coming with the student, it will need filled out and signed prior to the clinic. We highly recommend that a responsible adult is with the student as in normal circumstances, vaccinations would not be administered unless a parent is present.
Also attached is the vaccine information sheet that some may want to see prior to signing up for the vaccine. These will be handed out during the clinic as well."
Consent forms will be available for students to pick up in the High School office prior to the clinic if needed.
