Parent or Caregiver,

Your child’s school would like your feedback about walking and biking to school. This short survey will take about 5–10 minutes to complete. We ask that each family complete one survey per school their children attend. If you have more than one child in the same school, please complete the survey for the child with the next upcoming birthday.

All responses are confidential. Neither your name nor your child’s name will be linked to the results.

Thank you for taking the time to participate!
